Functional Wellness Rising Policies

Cash-Based Practice

In the state of California, naturopathic providers are not recognized by insurance companies for coverage. Therefore, Functional Wellness Rising operates as a cash-based practice. You can use a Health Savings Account (HSA) or Flex Spending Account (FSA) to pay for visits and supplements. Some diagnostic labs and imaging may be covered depending on your insurance plan. It is the patient's responsibility to contact their plan for information on coverage, benefits, eligibility, and more.

Financial Policy

Credit Card on File: After your first appointment, a credit card will be required to be kept on file in our secure billing software.

Automatic Charges: Your credit card will be automatically charged if a bill is unpaid after 30 days from the initial billing date or if an appointment is rescheduled with less than 24 hours' notice or is a no-show.

Security: Credit card information is encrypted and stored securely, not on the website. Policies are subject to the discretion of Functional Wellness Rising.

Cancellation & No-Show Policy

Cancellation Policy: A 24 business hours notice is required for cancellations. If an appointment is on Monday, we need to be informed by Friday before 2 PM. Cancellations made less than 24 hours in advance will incur a 50% charge.

No-Show Policy: If you do not show up for an appointment, 100% of the appointment fee will be charged to your credit card on file. Further appointments will not be scheduled until your balance is resolved.

On-Time Policy: Appointment lengths will not be extended due to late arrival. Please complete your intake and consent forms prior to your appointment. If forms are incomplete, they will be filled out with the doctor during your appointment, but the appointment length will not be extended.

Good Faith Estimate

You have the right to receive a "Good Faith Estimate" that explains how much your medical care will cost. Health care providers are required by law to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services before those items and services are provided.


Estimate Requests: You can request a Good Faith Estimate for the total expected cost of any health care items or services. This includes related costs like medical tests, prescription drugs, and equipment.


Scheduling and Estimates:

● If you schedule a health care item or service at least 3 business days in advance, you will receive a written estimate within 1 business day after scheduling.

● If you schedule at least 10 business days in advance, you will receive a written estimate within 3 business days after scheduling.

● You can request an estimate before scheduling, and it must be provided in writing within 3 business days after your request.


Dispute: If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. Save a copy or picture of your Good Faith Estimate for your records.
